Follow steps below to submit your service record:
1. Please sign into your account or create a new one
2. Select the person you are registering to receive the Volunteer Refund
3. Please fill out information of the Request Form
4. Upload your signed volunteer record (signed by Board Member or Team Manager)
No partial refunds will be granted (i.e. at least 10 volunteer points will be required to receive a refund).
